What is myHealth Online?

myHealth Online is an easy-to-use secure web site that gives you access to your health information. You can view your medications, test results, medical history, and immunizations, as well as access other services, such as appointment scheduling, messaging your care team and requesting a prescription renewal. Informative internet links provide additional details about your medications, health conditions, lab tests, and more. It also has tools that allow you to track and graph your lab results, such as cholesterol and blood sugar levels.

Santa Clara Valley Medical Center Hospital & Clinics offers myHealth Online as a free service. Internet access through a computer or a data-enabled smartphone is all you need to use the system.

Is information in myHealth Online Secure?

Yes. myHealth Online is secure. Your information is safe and protected. We adhere to strict Federal and State confidentiality laws, and we have implemented strong information security measures to protect your privacy.

How do I sign up for myHealth Online?

You must be a Santa Clara Valley Medical Center Hospital & Clinics patient aged 12 years or older to sign up for your own account. Parents or guardians of children younger than 12 can access records as a proxy (see Access to My Family's Records)

How do I get the myHealth Online app for my phone?

You can get myHealth Online with the MyChart® app on iTunes or through the Android Market.

Clinical Notes: What has changed?

Starting November 2, 2020, you can access more of your healthcare information through our myHealth Online application or the myHealth Online web portal. Notes gives you access to the health care notes that your doctors and other providers write during or after your visits in emergency department, urgent care clinic, and regular clinic, as well as hospital admissions and surgeries.

How do I know if I have a new note and where do I find it?

When your note is available, you will get an email letting you know that you have a new message in myHealth Online. When you open the new message in myHealth Online, it will let you know which visit has a new note available for you to read. How soon after an appointment will I be able to see my notes? Your note will be available when your care team has finalized all documentation.

Why can’t I see a specific note I expected to see in myHealth Online?

There are a few reasons why you may not see a note in myHealth Online:

  • The note may have been written before November 2, 2020, when note sharing started.
  • The note may not be ready. After your doctor makes the note final, it will become available for you to view.
  • Your doctor may have chosen not to share a particular note. Please contact your doctor for more information.
  • Notes and test results for adolescent patients between 12-17 years old will not be available in myHealth Online due to privacy concerns.

What if I have questions about the information in the note?

The note is part of your medical record. Doctors use the note to document your care and to communicate with other medical professionals. It may contain medical language, abbreviations or words that are not easy to understand. If you have questions about your note, write them down so that you can discuss them with your doctor at your next appointment. If you have an urgent question or concern, please contact your doctor’s office. If you wish to amend your medical record, please contact the Santa Clara Valley Medical Center Medical Records Department at 408-885-5125.

Can I change who has access to myHealth Online Chart?

If you already have given someone access to your chart but you no longer want to share your information including clinical notes, you can cancel or revoke their access. Go to ‘My Record’ then ‘Share My Records’ and there and you can revoke access that you have previously granted to someone.

What about my privacy and confidentiality?

Note sharing on myHealth Online will not change the confidential nature of the notes your clinicians write. Only you, those you choose to share your myHealth Online access with, and clinicians involved in your care, have access to these notes. If you do not want to see your notes online for a visit, ask your provider not to share your notes online. This request must be made on a visit-by-visit basis. To help keep your information secure remember to keep your myHealth Online login name and password private. You can change your password anytime by selecting “Profile” then “Security Settings” in the web-browser version of myHealth Online. To change your password in the MyChart mobile app, click on 3 vertical dots. Remember to always exit myHealth Online by selecting “Log Out” when you are done.

Viewing Test Results

Most test results will be available in myHealth Online the same day they are resulted. In some cases it may take longer depending on the type of test.

Messaging My Care Team

Now you can send messages to your primary and specialty care teams, request prescription renewals, and send appointment requests online. Any messages sent will become a permanent part of your medical record. These new features are available to adults and families with children under 12 years old.

Prescription Refill Requests

If you cannot manage your refill using myHealth Online, please call your pharmacy to order your refills. If you would like to transfer your prescription to a SCVMC pharmacy, please call 1-800-334-1000 (option 2) to reach our pharmacy staff who will manage the transfer for you. SCVMC Pharmacies can coordinate your refills when you sign-up for the Ambulatory Pharmacy Medication Refill Service.

Viewing Past and Upcoming Appointments

On the Appointments tab, you can review upcoming appointments and past visit summaries. You can also download and forward copies of these summaries to another provider.

Scheduling and Cancelling Appointments

You can schedule appointments with your primary care provider online. Most appointments can also be cancelled online. For those appointments which cannot be cancelled online, a message with the contact phone number will display so you can call and discuss changes to your appointment needs.

Please be courteous in providing at least 24 hours notice if you know you cannot keep an appointment. Such notice allows us to manage our schedules and offer unused appointments to other patients who may also be in need.

Access to My Family's Records

In order to view another person's health record through myHealth Online, you must be granted access as a proxy for that patient. The type of information shared depends on the relationship and ages of the patient and their proxies.

You may be a proxy for a patient whom you are responsible such as a child or dependent adult. Adult patient's may choose to give another adult permission to access their health information online. The person acting as proxy does not have to be a patient of Santa Clara Valley Medical Center Hospital & Clinics.

Authorization and Request forms are required to be completed and submitted to the patient's clinic in order to establish proxy access. For your convenience, you can download and print the forms below:

Logged-Out unexpectedly

Protecting your privacy and securing your health information is very important. Because of this, you will be automatically logged out of myHealth Online after 15 minutes of inactivity. We also recommend logging out whenever you need to leave your computer unattended.

Activation Code lost or expired

For your security, activation codes cannot be released over the phone. You can go to your clinic with a valid ID to request your activation code, or you can submit your request online. Activation codes expire after 180 days and are only valid for one-time use.

Activation Code does not seem to work

Please refer to the “Sign Up Now” section on the home page. Your code is valid only for one-time account setup. After initial setup, you will provide the username and password you created to access your account. If you need assistance to sign in, call 1-888-334-1000 and a representative will assist you.

Forgotten Username or Password

If you cannot recall your username or password, follow the Forgot Username? or Forgot Password? links on the home page. You will be led through steps to retrieve your username or reset your password. For your protection, your account will be inactivated after 5 failed attempts to enter the correct information.

Usernames must have at least 6 characters and are not case-sensitive. Passwords must have at least 8 characters and are case sensitive.
